• Work closely with physicians, nurse practitioners, and other healthcare professionals to develop and implement care plans, and communicate effectively with patients and their families.\n
• Educate patients on managing their conditions, medications, and lifestyle changes, and assist with discharge planning to ensure a smooth transition to home or other care settings.Qualifications : \n
