Job Summary :

We are seeking an experienced and detail-oriented Home Health and Hospice Scheduler to join our team. The ideal candidate will have extensive experience with WellSky / Kinnser software. This role involves scheduling patient visits, coordinating with healthcare professionals, and ensuring the efficient operation of our home health and hospice services.

Key Responsibilities :

Scheduling :

  • Schedule and coordinate patient visits for home health and hospice care.
  • Ensure optimal allocation of resources and staff availability.
  • Manage and adjust schedules to accommodate changes in patient needs and staff availability.

Coordination :

  • Communicate effectively with healthcare professionals, including nurses, therapists, and physicians.
  • Ensure timely and accurate communication of schedule changes to staff and patients.
  • Collaborate with the clinical team to ensure patient care plans are followed.

    • Software Management :

  • Utilize WellSky / Kinnser software for scheduling and documentation.
  • Maintain accurate and up-to-date patient records within the system.
  • Troubleshoot and resolve any issues related to scheduling software.

    • Qualifications : Experience :

  • Minimum of 2 years of experience as a scheduler in a home health or hospice setting.
  • Proficiency with WellSky / Kinnser software is required.
  • Knowledge of medical billing and coding is a plus.

    • Skills and Abilities :

  • Strong organizational and time-management skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Detail-oriented with a high degree of accuracy.
  • Proficient in Microsoft Office Suite.

    • Additional Requirements :

  • Positive and professional attitude.
  • Ability to multitask and manage multiple priorities.
  • Willingness to undergo a background check.
