Job Description\n

American Traveler is seeking an experienced RN Care Coordinator for a utilization assessment role, requiring a CO or compact RN license and strong care management skills.

\n \n Responsibilities\n

  • Position based in a utilization assessment department within a hospital setting
  • Typical patient population includes individuals with complex or chronic health conditions
  • Work schedule is 8 a.m. to 4 : 30 p.m. with weekday and rotating weekend shifts
  • No on-call duties are required
  • Floating may be required to facilities within a 60-mile radius
  • Uses Epic for electronic charting; prior Epic experience preferred
  • Conducts assessments and develops care coordination plans for patients with complex needs
  • May require home visits and outreach in nontraditional settings to support patient care
  • Collaborates across multidisciplinary teams to facilitate timely and appropriate care
  • Assists patients in navigating healthcare systems and accessing community resources
  • Uses evidence-based tools and quality measures to improve patient outcomes
  • Must follow flu season vaccination requirements with medical exemptions only as allowed
  • Local candidates within 60 miles are not eligible for travel positions

  • Active CO or compact RN license required at time of consideration
  • Completion of an accredited Registered Nursing Program required; BSN preferred
  • At least 3 years of recent RN experience, preferably in case management, utilization review, managed care, population health, or chronic disease management
  • American Heart Association (AHA) certifications required and must be current
  • Experience with complex care coordination and independent decision-making preferred
