Opportunity Description\n\nAs a Nurse Care Manager, you will play a vital role in care coordination, including in-home assessments, planning, facilitation, advocacy, and authorization of covered services to meet members' health needs while promoting quality, cost-effective outcomes.\n\nCompany Information\n\nOur client serves more than 18,000 frail and disabled individuals, empowering them to maintain independence and live safely in their own homes for as long as possible\n\nJob Duties\n
- Collaborate with members and their families to maintain the most independent living situation possible.\n
- Ensure consistent care across the healthcare continuum by assessing and closely monitoring members' needs and status.\n
- Authorize covered services and coordinate care across different payers.\n
- Communicate and collaborate with members, family members, caregivers, primary care practitioners, and the interdisciplinary team.\n\nExperience & Skills Required\n
- Current NYS RN License and registration\n
- Associate's Degree in Nursing Required. Bachelor of Science in Nursing (BSN) Preferred\n
- Minimum of 3 years of RN experience\n
- Clinical expertise in geriatrics preferred; managed long-term care experience is also a plus.\n
- Bilingual Spanish or Chinese preferred\n\nOther Information\n
- Salary Range : 105,000 - $115,000\n
- Bonus : Eligible for a 20,000 sign-on bonus ($5,000 payable every six months, less taxes)\n
