In this Care Advocate position, you'll wear many hats and work in a variety of environments. You'll get to act as an educator, evaluator, service coordinator, and community resource liaison as you help prepare members to transition back into the community. Your work will help promote community tenure by developing a system of support to address their health and wellness needs. If you are located in Washington, you will have the flexibility to work a hybrid schedule which will include both remote telephonic work and field based in-person care.

  • Approximately 10% of the week is spent in the community, meeting with members in hospitals, facilities, or homes. The remainder of the week is spent working remotely at home. Licensure and residency in Washington is required.

Primary Responsibilities :

  • Provide care management services to members which include coordination of care and behavioral health consulting
  • Serve Washington Medicaid members telephonically (primarily) with field responsibilities as required (typically within a 50 mile travel radius)
  • Provide behavioral health education, coaching and treatment decision support for members
  • Assess and interpret customer needs and requirements
  • Identify solutions to non-standard requests and problems
  • Assess current behavioral health status for members
  • Identify and prioritize gaps in care to develop plan of care to empower members to meet identified goals
  • Coordinate transitions between various areas of care from inpatient setting, state psychiatric facilities, and carceral facilities
  • Obtain information from providers and coordinate services amongst provider group
  • Attend case staffing's and inter-agency meetings supporting transitions in care settings facilitating pre- and post-release care plans

    Required Qualifications :

  • Master's degree in Psychology, Social Work, Counseling, or Marriage and Family Counseling; OR Registered Nurse w / 2+ years of experience in behavioral health
  • One of the following active, unrestricted, independent licenses to practice in the State of Washington : LICSW, LMFT, LMHC, PSY; OR RN with 2+ years of behavioral health experience
  • 2+ years Case Management experience providing behavioral health care coordination
  • Proficient in Microsoft Office Suite (Word, Outlook, and internet)
  • Residence and clinical licensure in the State of Washington
  • Dedicated workspace and access to high-speed internet via cable / DSL / fiber service in your home
  • Ability to Serve Washington Medicaid members telephonically with field responsibilities as required
  • Preferred Qualifications :

  • Dual diagnosis experience with mental health and substance abuse either in an outpatient or inpatient setting
  • Carceral behavioral health service delivery experience
  • Experience working in an environment that required coordinating benefits and utilizing multiple groups and resources for patients
  • Experience working with the Medicaid patient population

  • Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $58,800 to $105,000 annually based on full-time employment. We comply with all minimum wage laws as applicable.
