Nurse Practitioner – Primary Care Clinic

Location : Modesto, CA

Employment Type : Full-Time, Direct-Hire

We are seeking a dedicated Nurse Practitioner to join a well-established employer-based health clinic in Modesto. This onsite primary care clinic provides care for employees and their families, offering a unique opportunity to work in a collaborative, patient-centered setting. About the Role

This is a permanent, full-time position with the option to start as contract-to-hire. The Nurse Practitioner will deliver high-quality primary care services to patients ages 2 and older. The clinic offers a broad range of care, from acute visits to wellness and preventive services, in a supportive and collegial environment.

Key Responsibilities

Provide comprehensive primary care, health assessments and ongoing patient management

Deliver care to a diverse population including pediatrics, adults and older adults

Collaborate with a multidisciplinary team to ensure patient-centered outcomes

Support occupational health needs as required, including triage and screenings

Document in EPIC EMR system

Qualifications

Current California Nurse Practitioner license with 103 NP designation required

DEA license (or willingness to obtain within 60 days)

Minimum of 1–2 years of primary care experience

Experience with EPIC EMR and occupational health preferred

Strong clinical assessment, diagnostic and patient education skills

Schedule & Employment

Full-time, Monday–Friday
