What You'll Do as a Hospice RN : \n
• Deliver palliative, end-of-life care to clients in their homes and / or senior living facilities\n
• Follow individualized care plans tailored to each patient's needs\n
• Administer medications, treatments, wound care, and more\n
• Monitor conditions and report changes to the clinical team\n
• Coordinate care and services across disciplines\n
• Document all visits and updates using our intuitive tablet-based EMR\n
• Travel to client homes within the assigned territory\n\n? What You'll Need : \n
• Current RN license in New Jersey\n
• Minimum 1 year of recent, verifiable clinical experience (hospital, med-surg, etc.)\n
• Hospice experience is preferred\n
• Graduate of an accredited nursing program\n
• Strong interpersonal and time management skills\n
• Ability to work independently and travel locally\n
