Job Title : Associate Patient Care Coordinator

Reliant Medical Group (Primary Care for Adult Medicine) has an immediate opening for a friendly, patient focused and detail oriented Associate Patient Care Coordinator to join our team. The Associate Patient Care Coordinator is responsible for the completion of set processes and protocols. Works cooperatively with all members of the care team to support the vision and mission of the organization, deliver excellent customer service, and adhere to Lean processes. Supports the teams in meeting financial, clinical, and service goals.

Location : 378 Maple Ave Shrewsbury, MA

Hours : Monday through Friday, between the hours of 7 : 30am - 5pm EST. Schedule will be determined upon hire.

Primary Responsibilities :

• Primarily call center environment in a patient clinic team-based setting
• Handling phone incoming and outgoing phone calls
• Message management in EMR (electronic management record) system
• Providing direct administrative support to the practice and within a care team
• Scheduling appointments
• Organizing and completing patient forms and letters
• Updating patient information
• Insurance verification
• Obtaining prior authorizations for exams / testing
• Collaboration with medical providers, nurses, medical assistants and ancillary staff
• Problem solving patient issues / requests
• Providing coverage for check-in / check-out patients

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $16.00 to $27.69 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
