Description :

This role brings a gentler, more effective treatment to the geriatric population, which vastly improves their quality of life.

We work with skilled nursing facilities to design and build customized home dialysis dens within the building.

This first-to-market approach benefits all involved.

Patients receive shorter, gentler, and more frequent treatments, more closely mimicking the function of healthy kidneys.

Patients experience decreased recovery times and a dramatic improvement in their quality of life.

Our nurses are highly trained with a specialized skill set in dialysis, resulting in exceptional levels of compassionate care.

Key responsibilities include functioning at a high level with little oversight to provide nursing care, training, and home support as needed.

You will work collaboratively with team members, facility representatives, and dedicated nephrologists.

You will collect and record relevant patient health data before, during, and after hemodialysis treatment.

Developing and maintaining a plan of care that describes interventions to attain expected outcomes is essential.

Implement and record interventions to achieve expected outcomes in electronic medical records.

You will perform laboratory collection, processing, and shipping procedures, and reschedule missed or inadequate laboratory collections as needed.

Occasional travel to nearby locations will be required.

Medication administration and documentation are also part of the role.

Experience with NXStage dialysis machines is strongly preferred.

Geriatric patient experience is preferred, and CNN or CDN certification is also preferred.

Crown Web EMR experience is preferred.
