About The Position

Job Title : Registered Nurse

License : Must have license in hand

Certifications : CPR, BLS

Must-Have : Recent specialty experience, proficiency in HomeCare HomeBase EMR and OASIS

Description :

Provides, coordinates, and directs the provision of home nursing care according to physician's orders based on agency policies and procedures.

Responsible for the competent application of the nursing process.

Supervises and evaluates Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.

Home visits and periodic office meetings are required.

Must have reliable transportation, a current driver's license, and agency-required liability insurance.

Average number of patients per day is 5-8.

Contractor will be assigned between 5-10 patients per day.

Each visit and activity holds a different point value.

Types of cases include Start of Care, Wound Care, and Follow-up Visits.

Must be willing to travel up to 75 miles from the home clinic.

Patients will be visited in homes as well as care facilities.

This position may involve on-call responsibilities.

All documentation must be completed within 24 hours of the appointment with the patient.

Late documentation that impacts billing and follow-up patient care may result in a cancellation.

There are no guaranteed hours for this contract.

Clinicians must hit weekly productivity points to meet expectations.
