This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needsBenefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insuranceA 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates HOME HEALTH REGISTERED NURSE | RESPONSIBILITIES : Assess and implement individualized care plans.Observe, plan, implement, and evaluate nursing support for individuals with acute or chronic conditions.Supervise care plans and daily work assignments of supporting staff.Coordinate communication between healthcare professionals to address patient needs.Manage medication processes, including ordering, monitoring, and documentation, in compliance with policies.Maintain accurate nursing records and progress reports.Provide health and safety training for new employees and ongoing recertifications.Ensure unlicensed staff are trained on person-specific health needs.Support care transitions and collaborate with discharge coordinators as needed.

HOME HEALTH REGISTERED NURSE | REQUIREMENTS : Education : High School Diploma or GED (Verifiable).Attendance : Commitment to mandatory attendance during the first 90 days.Active Indiana RN license or compact state license.Home health and OASIS experience strongly preferred.Strong organizational and communication skills.Ability to work independently and as part of a team.
