Job Summary :

We are seeking an experienced and detail-oriented

Inpatient Pharmacist

to join our hospital pharmacy team. The Inpatient Pharmacist will be responsible for ensuring the safe, effective, and cost-efficient use of medications across all inpatient care areas. The ideal candidate will possess strong clinical judgment, a solid understanding of hospital pharmacy operations, and a commitment to patient-centered care.

Key Responsibilities :

Review and verify medication orders for accuracy, appropriateness, and potential drug interactions.

Prepare, compound, and dispense medications in accordance with state and federal regulations.

Provide clinical pharmacy services, including therapeutic drug monitoring and pharmacokinetic dosing.

Collaborate with physicians, nurses, and other healthcare professionals to optimize medication therapy and patient outcomes.

Participate in interdisciplinary rounds, offering evidence-based recommendations on drug therapy.

Monitor for adverse drug reactions and medication errors, and report per hospital policy.

Ensure compliance with all

Joint Commission ,

State Board of Pharmacy , and

hospital policies .

Oversee the preparation and delivery of IV admixtures, including antibiotics, chemotherapy, and parenteral nutrition.

Educate patients and staff regarding safe and effective medication use.

Maintain accurate records and documentation within the

Electronic Medical Record (EMR)

and pharmacy systems (e.g.,

Epic ,

Cerner ,

Pyxis , or

Omnicell ).

Participate in quality improvement initiatives and formulary management activities.

Requirements :

Doctor of Pharmacy (Pharm.D.)

degree from an accredited institution.

Active Pharmacist license

in the state of practice (or eligibility to obtain prior to start).

Minimum

2 years of inpatient hospital pharmacy experience

required .

BLS certification

(AHA or ARC) required.

Strong knowledge of pharmacology, sterile compounding, and hospital medication systems.

Excellent communication, analytical, and problem-solving skills.
