Description :
The Clinical Nurse is a Registered Professional Nurse who provides safe, competent quality care based on nursing theory and research to a designated group of patients and significant others.
Delivers patient care by incorporating the tenets of Relationship Centered Care and through the application of the nursing process based upon theory, research, and evidence-based practice.
Assesses and evaluates patient care needs and applies critical thinking skills in patient care management.
Integrates relevant assessment and intervention skills in the delivery of nursing care.
Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care.
Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care, delegating to others as appropriate.
Educates patients and caregivers about their plan of care, transition of care, promotion of health, and prevention of disease while anticipating needs and readiness to learn.
Communicates effectively and professionally with patients, family, and all members of the Interdisciplinary Patient Care Team.
Manages assignments within the Care Delivery Model and demonstrates effective allocation of material resources.
Models the tenets of Relationship Centered Care through facilitation of various actions including admission welcome, bedside shift report, care management, and purposeful rounding.
Considers the patient's values, preferences, cultural diversity, expressed needs, and knowledge in all aspects of care.
Establishes and maintains a therapeutic relationship with the patient and family.
Uses evidence-based practices to enhance understanding of patients' perceptions of care.
Initiates service recovery for patients and families with concerns and escalates when needed.
Demonstrates caring, respect, compassion, empathy, and active listening in all interactions.
Supports staff in using evidence-based practices to improve understanding of patients' perceptions of care.
Conducts purposeful hourly rounding on assigned patients.
Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement.
Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served.
Implements process improvement strategies based on evidence-based procedures.
Engages in peer and institutional review processes for quality assurance.
Collaborates with interdisciplinary teams on quality improvement projects and research.
Contributes to quality and safety practices and compliance measures.
Implements principles of high reliability to avoid high-risk behaviors, ensuring patient safety.
Escalates potential safety hazards or gaps from best practice following established protocols.
Utilizes information technology for documentation to support quality and performance improvement initiatives.
Performs charge nurse duties as assigned.
Coordinates with management to ensure supplies are appropriately utilized and inventory is sufficient.
Escalates resource deficits to appropriate leadership.
Demonstrates patient-focused and cost-effective approaches to care.
Documents accurately and thoroughly in compliance with regulatory and standards of care.
Facilitates patient throughput on a shift basis.
Contributes to the environment of care supporting colleagues, patients, and families.
Advances clinical competence in nursing practice.
Projects a professional image and communicates effectively with respect and compassion.
Incorporates ethical principles into decision-making processes.
