Georgia Licensed Care Manager (RN)

A company is looking for a Care Manager (RN) to develop and facilitate care management activities for members with physical health needs.

Key Responsibilities

Evaluate member needs and recommend care plans to address barriers and social determinants of health

Coordinate care between members, families, and providers to ensure person-centered services

Monitor member progress and update care plans as necessary to meet individual goals

Required Qualifications

Degree from an Accredited School of Nursing or Bachelor's degree in Nursing

2 - 4 years of related experience in care management

RN - Registered Nurse - State Licensure and / or Compact State Licensure required

Must reside in the state of Georgia
