Job Description : Physical Therapist

Performs physical therapy evaluations, establishes plan of care to meet identified goals, provides treatments, evaluates effectiveness of treatments, and modifies plan of care as needed to achieve desired outcomes.

This Physical Therapist position will work 4-10's a week and will perform physical therapy evaluations, establishes plan of care to meet identified goals, provides treatments, evaluates effectiveness of treatments, and modifies plan of care as needed to achieve desired outcomes.

EDUCATION / CERTIFICATIONS / LICENSES / DEGREES

• Licensed as a Physical Therapist in the State of Oregon
• American Heart Association Healthcare Provider Basic Life Support (BLS)
