Social Worker, Hospice & Palliative Care – Full-Time | Central Wisconsin

A hospital near La Crosse, WI is seeking a compassionate Social Worker to join their Hospice & Palliative Care team!

Social Worker, Hospice & Palliative Care Care Schedule :

  • Full-Time | Direct Hire
  • Monday – Friday | 8 : 00 am – 4 : 00 pm

Social Worker, Hospice & Palliative Care Responsibilities :

  • Provide psychosocial assessments, counseling, and support for patients and families
  • Collaborate with an interdisciplinary team to develop and update individualized care plans
  • Educate patients, caregivers, and families on palliative care, comfort measures, and the dying process
  • Maintain accurate documentation, uphold ethical / legal standards, and participate in team meetings and quality initiatives

    • Social Worker, Hospice & Palliative Care Qualifications :

  • MSW from an accredited School of Social Work
  • Wisconsin Certificate from the Department of Regulations & Licensing Examination Board of Family Support Coordinators
  • Minimum of 3 years of clinical experience
  • Completion (or agreement to complete) hospice / volunteer training
