Wound / Infection Ctrl Nurse

Must be certified in Infection Control

Previous experience in Wounds and Infection Control working in a LTC facility.

CT Licensed RN

MDS knowledge, Care Planning and assessment knowledge.

Must be able to do wound rounds with Dr every Monday and follow up with physician ordered patient wound care following the wound care guidelines.

Assesses residents to determine their risk factors for contractures, skin breakdown and deterioration in ADL's in order to develop a nursing rehab plan of care.

Develops a plan of care for each resident with current pressure sore, or evaluated as a high risk for development of pressure sores.

Completes wound care, positioning audits, analysis of trends etc.

Monitor EMR dashboards "alerts" pertaining to wound assessments.
