The position is contract-based with a standard weekday schedule. Proficiency in generic office software is necessary. Requirements Graduation from an accredited nurse practitioner program. Minimum of two years of experience in clinical nursing practice. Minimum of one year of experience with frail or elderly populations. Licensed to practice as a Nurse Practitioner and Registered Nurse in NY. DEA Certification. Basic computer skills in office software. Bilingual proficiency preferred. Qualifications Experience in home care or care management. Effective communication and interpersonal skills. High-level analytical and research skills. Ability to manage multiple projects and meet deadlines. Board Certification by AANP or ANCC. Medicaid and Medicare Provider IDs in good standing. Medically cleared for communicable diseases and up-to-date immunizations.
