- Deliver high-quality home health nursing visits across varied patient needs.
- Handle cases such as Start of Care, Resumption of Care, Wound Care, Pediatrics, and Recertifications (higher pay for complex visits).
- Collaborate with physicians and care teams for seamless continuity of care.
- Document visits via tablet within 24-48 hours.
- Stay compliant with DOH regulations while delivering compassionate, culturally sensitive care.
- ? What You'll Need
- Active New York RN License
- Reliable transportation + valid driver's license
- Experience with OASIS assessments (preferred)
- Home care or acute care experience (preferred)
- Knowledge of Homecare Homebase (HCHB) is a plus
