Washington Licensed Pediatric Case Manager

A company is looking for a Pediatric / NICU Case Manager, RN - Remote Living in WA.

Key Responsibilities

Engage members to complete comprehensive needs assessments addressing medical, behavioral, and socioeconomic needs

Develop and implement person-centered care plans in alignment with evidence-based guidelines

Collaborate with care teams and community resources to support member self-management and coordinate care transitions

Required Qualifications

Current, unrestricted RN licensure in the state of Washington

2+ years of NICU and / or Pediatric clinical experience

1+ year of experience with MS Office applications

Reside in the state of Washington

Designated workspace with secure high-speed internet access
