Job Description for Licensed Practice Nurse (LPN)

We are looking for a dedicated LPN to join our team who will see and manage the clinical patients for both our skilled and non-skilled divisions, ensuring exceptional patient care and compliance with healthcare regulations and our policy and procedure. The ideal candidate will provide high-quality nursing care to clients in their homes, ensuring their comfort and well-being. S / he will support each clients unique goals and ensure each client is well cared for, and feel truly seen, and heard.
This role requires strong foundation in medical documentation, a thorough understanding of physiology, and the ability to work independently while managing multiple patient needs.
Duties and Responsibilities :
- Travel to clients homes to provide patient care and implement the plan of care initiated by the RN
- Provide accurate and timely documentation of all interactions with clients, care provided and any changes in health status after each visit as consistent with the plan of care
- Assess and provide patient and family / caregiver education and information pertinent to the diagnosis and plan of care
- Participate in the coordination of home healthcare services, appropriately reporting identified needs for other disciplines (HHA, OT, PT, MSW, ST) to the clinical manager
- Administer medications and treatments as prescribed and participate in the peer consultation process, communicate with the clinical manager and other clinicians on the team to provide updates, insights, and recommendations for clients progress
- Actively participate in a high-performance environment to not only drive change but truly enhance the quality of client service
- Provide support for various specialties including geriatrics, post-surgical care, post-stroke care, cardiac care, diabetic care, and renal care
- Assist with laboratory tests and procedures when required
- Participate in personal and professional growth and development
- Document all clients interactions
Qualifications :
Graduate of an accredited practical nurse or vocational nursing program and is currently licensed as an LPN in the State of VAMinimum of one (1) year experience in a home healthcare or private duty settingStrong leadership, communication, and organizational skillsAbility to work independently and collaboratively with a diverse teamMust be a licensed driver with an automobile in good working conditionPossesses and maintains a current CPR certificationKnowledge of healthcare regulations and standards and ability to maintain compliance with such regulations and standards of practiceExperience :
Proficiency in medical documentation software. Experience in AloraPlus is an advantageKnowledge of physiology relevant to patient careProficiency in disease management, medication management / teaching / administration, postoperative management, pain management, infusion therapy, lab draws, tracheostomy care, foley catheter care, nasogastric tube care, intravenous line insertion and care, wound care and wound V.A.C. care, measurement of vital signs (blood pressure, pulse, respiration rate, lungs sounds, pulse oximetry, and glucose monitoring)Why Work with Us :

Pay Rate : $30.00 $60.00 / hr and / or $70.00 $120.00 per visit. Rates are negotiable based on applicants experience
