Description

  • Conduct evaluations for patients in both inpatient and outpatient settings while formulating personalized treatment plans
  • Deliver physical therapy interventions targeting mobility, strength, balance, and functional skills
  • Work collaboratively with fellow health professionals including nurses, physicians, case managers, and other therapy specialists to enhance patient outcomes
  • Provide education for patients and their families regarding therapeutic goals, discharge plans, and home exercise regimens
  • Document evaluations, treatment progress, and patient outcomes in line with facility policies and regulatory standards
  • Engage in multidisciplinary rounds and care conferences to guarantee coordinated and seamless care

Education

  • Doctorate in Physical Therapy (DPT) or its equivalent from an accredited program
  • Certifications

  • Active state licensure as a Physical Therapist
  • CPR certification is preferred
  • Additional specialized certifications (such as in geriatrics or neurology) are a plus but not mandatory
  • Skills

  • Previous experience is highly preferred
  • Proficient in treating patients with intricate medical and mobility challenges
  • Excellent communication and collaboration skills
