A confidential long-term care and rehabilitation center in Oceanside, NY is urgently hiring a Licensed Practical Nurse (LPN) Floor Nurse to join its patient-focused team.

Key Responsibilities

As an LPN Floor Nurse, you will :

Provide direct patient care while maintaining the highest standards of safety and infection control.

Administer medications and perform treatments as prescribed.

Accurately document patient admissions, transfers, discharges, and ongoing care in electronic medical records.

Monitor patient conditions, communicate changes to physicians, and adjust care plans as needed.

Educate patients and families about therapies, rehabilitation regimens, and health adjustments.

Collaborate with physicians, RNs, and interdisciplinary teams to ensure optimal patient outcomes.

Uphold confidentiality standards and adhere to Department of Health regulations and facility policies.

Qualifications

Licensure : Active and unexpired Licensed Practical Nurse (LPN) license in New York.

Certification : MediClear or equivalent healthcare credential required.

Education : Completion of an accredited nursing program.

In-depth knowledge of infection control measures, patient care protocols, and healthcare regulations.

Strong communication and teamwork skills.

Ability to manage multiple priorities while maintaining accuracy and a high quality of care.
