  • Develop and implement individualized care plans based on current and prior medical history\n
  • Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding\n
  • Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey\n
  • Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure\n
  • Administer physician-ordered medications and treatments with skill and empathy\n
  • Educate patients and their families on care techniques and symptom management\n
  • Track and communicate changes in patient condition, ensuring timely and effective responses\n
  • Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided\n
  • Collaborate with interdisciplinary teams to ensure comprehensive care delivery\n
  • And contribute to other supportive duties as part of a compassionate, mission-driven team\n

  • A heart for hospice care and a commitment to exceptional service\n
  • A valid Registered Nurse license in good standing in the state you will work in\n
  • A valid driver's license and reliable transportation\n
  • CPR certification (current)\n
  • Strong observational and communication skills—both written and verbal\n
