IDR Healthcare is seeking a local contract Home Health Physical Therapist for a local contract job in Henderson, Tennessee.

Position : Physical Therapist (PT) – Home Health

Location : Henderson, TN

Start Date : ASAP

Contract Type : Contract

Schedule : Monday–Friday, 8 : 00 AM – 4 : 30 PM

On-Call : PRN availability for orthopedic patients (as needed)

Join a collaborative and mission-driven home health team dedicated to helping patients restore function and mobility in the comfort of their homes. This is a field-based role supporting a diverse caseload and working closely with interdisciplinary teams and referring providers.

🧠 Responsibilities & Role Overview

  • Conduct comprehensive physical therapy evaluations based on referrals and physician orders
  • Review medical records, history, and contraindications to design individualized treatment plans
  • Establish measurable goals and update care plans based on patient progress
  • Document evaluations, treatments, and outcomes per state / federal regulations and company timelines
  • Communicate patient needs and progress to family, physicians, and care teams
  • Recommend and implement use of adaptive equipment when appropriate
  • Supervise PTAs and Aides as per TN state practice act
  • Participate in staff development and continuing education to maintain licensure and professional standards

    • ✅ Requirements

  • Active Physical Therapy license in the state of Tennessee
  • Recent home health experience required
  • Familiarity with HomeCare HomeBase (HCHB) preferred
  • AHA BLS certification
  • Ability to work independently and manage a varied home health caseload
  • Strong interpersonal and documentation skills

    • Mileage reimbursed at $0.41 / mile

    Field-based position covering the local Henderson area

    Scrub color not specified – professional attire recommended

    Candidates must be dependable, flexible, and resilient in a home-based care setting

    PRN availability expected for orthopedic patient care
