The role you'll contribute :

Assumes a supportive role and has responsibility and accountability for facilitating, communicating, and collaborating with both the healthcare team, and the patient / family to identify and meet the physical, emotional and spiritual needs of the patient. Demonstrates personal accountability for relationship-based care, organizational mission, and core values. Provides direct patient care. Responsible for leading and providing front line staff support as it relates to day-to-day clinical operations. This includes, but is not limited to the delegation of duties to office staff as assigned, management of operational issues, assurance that staff adhere to clinic quality, regulatory, billing, and customer service standards, and the provision of staff duties. Individual will serve as point of contact for any issue in the daily operation of the clinic and will assess, resolve and / or escalate to Clinic Supervisor and / or Assistant Administrator as appropriate.

  • Assesses patients physical, psychosocial, cultural, and spiritual needs through observation, interview, review of medical records, and clinical data.
  • Evaluates patients response and reports to responsible provider to ensure optimal patient outcome.
  • Contributes to the development and implementation of patient care plans in collaboration with the interdisciplinary team.
  • Demonstrates competency in therapeutic and diagnostic interventions, including vital signs, minor treatments / procedures, prevention services, pre-visit planning, post-visit follow-up, and medication refills per protocol.
  • Promotes, advocates, and collaborates to protect the health, safety, and rights of each patient.
  • Provides patient and family education through assessment of health literacy, barriers to learning, readiness, needs, and ability.
  • Encourages patient engagement in healthcare partnership using motivational interviewing, evidence-based self-management support protocols, tools, and teach-back methodology.
  • Aids in the coordination of the patients care plan.
  • Recognizes the patient or designee as a full partner in providing compassionate and coordinated care based on respect for the patients preferences, values, and needs.
  • Provides relationship-based care, understanding what is most important to the individual and actively engaging them in all aspects of care.
  • Contributes to continuous process improvement by taking an active role in patient experience and satisfaction.
  • Other duties as assigned.
  • Oversees clinic staff as assigned, ensuring compliance with policies and procedures, preparing work schedules, ensuring adequate staffing, resolving clinical operational problems, managing clinical supply ordering and expenses, and conducting performance reviews.

Qualifications

  • High School Grad or Equiv Required
  • Technical / Vocational School Preferred
  • One or more years of Work Experience Preferred

  • American Medical Technologists (AMT) Required or
  • National Center for Competency Testing -(NCCT) Required or
  • National Healthcareer Association (NHA) Required or
  • American Registry of Medical Assistants (ARMA) Required
