IP coverage of both sites; APP will cover for provider. Patient Contact Hours : 6 Clinic days : 5 Clinic Hours : 8-5 Telehealth : Yes Number of Physicians : 3 Number of advanced Practice Providers : 3 Supervision of APP expected : Yes Nursing support / Physician Nurse Ratio : Non-nursing roles Ancillary Staff per provider : 0.5 (scheduler, biller, CNA) Number of Exam rooms per provider : 2 Medical Imaging : Yes Lab & Pharmacy : Yes Services / Procedures offered in the practice : Patient assessments, new treatment plans, ongoing treatment plans, changes in therapy, referral for ancillary services, end of life care, chemotherapy management, pain management (radiation and surgical oncology management provided by existing providers with strong relationships) Clinic Provider daily volume : 10-30 Average Daily Hospital Practice Census : 10-15 Primary, 5-8, Secondary Appointment Length : 20 mins. Patient Demographics : Primary Clinic; Medicare / Medicaid / Commercial; Secondary Clinical; Commercial, Medicare EMR : AllScripts- moving to Epic in late May 2025 Teaching Opportunity : Informal
