Field Case Manager - Registered Nurse
HealthCare Support is seeking a Field Case Manager - Registered Nurse in Minneapolis and surrounding areas to work with a statewide community dedicated to helping people with disabilities and older adults thrive in their homes.
Pay Details : $34 / hr. - $43 / hr.
Shift Details : Monday - Friday, 8 : 00 AM - 4 : 00 PM I NO On-Call!
Daily Responsibilities for Field Case Manager - Registered Nurse :
- 3-4 visits per day - assessing clients, creating care plans, and setting up home and community-based direct care services for clients with complex health needs as determined by their assessment
