Position Overview

We are seeking a Board-Certified Psychiatric Mental Health Nurse Practitioner (PMHNP-BC)

to join our team. The role will primarily involve rounding at skilled nursing facilities and

assisted living facilities, with an opportunity for cross-training into inpatient geriatric

psychiatry.

Responsibilities

Provide comprehensive psychiatric evaluations and medication management

Conduct ongoing follow-up visits with geriatric and adult psychiatric populations

Collaborate with interdisciplinary teams, including medical providers, nursing staff, and

social workers

Maintain accurate and timely documentation in compliance with regulatory and facility

standards

Contribute to continuity of care planning and patient / family education

Qualifications

Psychiatric Mental Health Nurse Practitioner Board Certified (PMHNP-BC)

Active or eligible for Ohio state licensure

Strong interest in geriatric psychiatry and behavioral health in long-term care settings

Excellent communication and teamwork skills

Prior experience in SNF, ALF, or inpatient psychiatry is a plus, but not required
