Hospice CNA

Description

Wellspring Lutheran Services is a statewide nonprofit dedicated to serving seniors, families and children throughout Michigan. We are currently seeking a Hospice CNA  to join our hospice team in Frankenmuth. In this role, you will provide compassionate, person-centered care to patients and families in-home and in skilled nursing facilities, supporting their comfort, safety and quality of life.

Responsibilities

As a Hospice CNA with Wellspring, you will :

• Provide personal care and services as outlined in the Plan of Care (POC), including bathing, grooming, dressing, feeding and mobility assistance.
• Observe, document and report changes in patient condition to the RN or appropriate skilled professional.
• Monitor vital signs (temperature, pulse, respiration, blood pressure) and recognize changes in patient status.
• Provide skin care to prevent breakdown and report concerns.
• Assist with toileting, incontinence care, catheter and colostomy care, enemas, and other tasks as competency allows.
• Support patients with exercises and range-of-motion activities as directed.
• Assist in medication administration as directed, following agency protocols.
• Maintain a clean, safe, and comfortable patient environment; perform light housekeeping and meal preparation as needed.
• Follow infection control procedures, universal precautions and organizational policies.
• Attend required trainings and educational sessions.
• Communicate effectively with patients, families, and team members, demonstrating professionalism and ethical standards.
• Respond to agency communications promptly for scheduling and updates.
• Recognize emergencies and initiate appropriate emergency procedures as needed.
