Description :

The Nurse Navigator plays a critical role in guiding patients through their healthcare journey, ensuring they receive comprehensive care and support. This position involves collaborating with healthcare teams to facilitate and handle all oral oncolytic medications for patients. The Nurse Navigator is responsible for monitoring and controlling the patients who are receiving medications and ensuring proper protocol while providing a positive patient experience.

Responsibilities

Will coordinate medication services processed to patients; which will include all prior authorizations.

Responsible for relaying details and instructions to ensure smooth transitions for patients from hospital to home or other care settings.

Ensure accurate coding for medical records.

Provide education to patients regarding treatments and general health management.

Utilize various systems to track patient progress and maintain accurate documentation.

Skills

Strong ability to obtain Prior Authorizations for medications.

Knowledgeable in outpatient care processes and discharge planning protocols.

Experienced in medical coding practices.

Familiarity with healthcare systems for effective patient tracking and documentation.

Strong communication skills to educate patients on their health journey.

Compassionate approach to care with a focus on patient advocacy.

Registered RN with at least 5 years of experience

Prior Authorizations experience required (nearly 80% responsibility engagement)

Requirements :

Ability to work collaboratively within a team-oriented environment while managing multiple priorities.

Nurse Navigator will be provided opportunity to attend 1 or 2 conferences per year around APC and Nurse Navigation.

In person office work is required as well as meeting with patients directly at our Pine Ridge office location in Naples. Ability to travel and meet with patients in person is required 100%.
