Responsibilities include : assessment, coordination, implementation and evaluation of the continuum plan of care in collaboration with other members of the multi disciplinary team and promotes the organizational commitment to customer service.\n\nII Position Qualifications : \n\nMinimum Education, Licensure / Certification and Experience Required.\n\nA. Education\n\nGraduate of an Accredited School of Nursing or equivalent.\n\nB. Licensure / Certification\n\nOncology Certified Nurse (OCN), preferred.\n\nCurrent licensure to practice nursing in the State of Michigan.\n\nCurrent Basic Life Support (BLS) certification.\n\nC Special Skill / Aptitudes\n\nAbility to make professional nursing judgments in following physician orders while working within the guidelines of established nursing policies and procedures of the assigned practice site.\n\nPositive interpersonal communication skills in order to effectively instruct and provide education to patients and family and maintain cooperative working relationships with co workers.\n\nExcellent customer service orientation skills necessary in order to deal effectively with various levels of\n\nhospital personnel, outside customers and community groups.\n\nD. Experience\n\nThree years of RN experience with two years of experience in oncology .\n\nIII Duties / Responsibilities : \n\n1 Assesses patients for immediate and post-discharge needs in conjunction with the patient care team and the patient / family, in accordance with SJM-O policies\n\n2 Provides patient care that reflects a respect for patients' rights, dignity, values, culture preferences, and expressed needs and an attention to providing comfort, relieving pain, and alleviating anxiety.\n\n3 Conducts a utilization management assessment on a visit basis to facilitate appropriate use of health care resources and coordination of care.\n\n4 Collaborates with patient / family, physician, hospital staff, and community agencies to develop discharge plans that prepare patients for continued care needs.\n\n5 Implements care in accordance with the plan, or clinical protocol appropriately utilizing care team members in the care delivery process.\n\n6 Identifies variances from clinical protocols, resolves or refers issues as appropriate, and identifies opportunities for the improvement of care.\n\n7 Responds appropriately to changes in workloads, patient census and / or staffing levels.\n\n8 Assess patient / family needs for information and education across the continuum, plans and implements patient teaching using a variety of techniques and methods, and evaluates effectiveness of educational interventions.\n\n9 Administers ordered medication and treatments as ordered by physicians.\n\n10 Monitors patients for side effects of treatments, coordinates with physician and implements supportive care measures.\n\n11 Performs phlebotomy and laboratory on-site testing. According to policy and procedure records test results and communicates test results to the physician.\n\n12 Responsible for patient medical record documentation in accordance to policies and procedures.\n\n13 Triage patients, follows established protocols. Assures appropriate advice is given to patients and families. Assures managed care guidelines are adhered to in terms of patient access. As required, reinforces discharge education with patients according to the direction of the physician.\n\n14 Participates in staff meetings, education in service programs and completes required continuous training and education.\n\n15 Implements quality control programs by reviewing and logging all quality controls and acting appropriately when results read out of control limits.
