1 CM to 16-20 patients

General Information Minimum Experience : 1.5 years general nursing (system requirement) Will you accept a first-time traveler? Yes, only if greater than 2 years of experience

Previous discharge planning and Acute Care Case Management experience, progression of care, experience meeting with families and discussing plans of care

Required Certifications : BLS
