Position Overview

HumanEdge Health is seeking a dedicated Physical Therapist for a 13-week travel assignment in Lewistown, Missouri. This position is within a Rehab / LTC setting providing skilled care to patients with physical functioning disorders and wound care needs. The role focuses on patient-centered, evidence-based rehabilitation to maximize independence and functional outcomes.

Responsibilities

Screen, evaluate, and rehabilitate patients with physical functioning and / or wound care disorders

Develop and implement individualized treatment plans per regulatory and clinical standards

Administer interventions including manual techniques, ambulation, therapeutic exercises, modalities, assistive devices, and wound care

Assess and document patient progress, adjusting treatments as needed

Evaluate treatment outcomes and modify services accordingly

Provide consultation and counseling to patients, families, and caregivers

Communicate effectively with supervisors and interdisciplinary team members regarding patient progress and care plans

Complete comprehensive discharge summaries and maintain accurate electronic documentation

Accurately record treatment minutes for all services provided

Participate in required meetings, in-services, and facility trainings

Obtain necessary DME or supplies per clinical requirements

Supervise and support PT assistants, rehab aides, or students

Adhere to APTA and state professional practice acts, codes of conduct, and ethical standards

Requirements

Minimum 1 year PT experience – Required

LTC or Rehab experience – Required

Missouri State PT License – Required

New grads accepted with Level II fieldwork experience
