KPG Home Health is seeking a skilled and compassionate Home Health Registered Nurse to join our dynamic team. As a Home Health RN, you will be crucial in providing quality healthcare visits to patients in the comfort of their homes.

Responsibilities :

Conduct patient assessments and develop individualized care plans.

Administer medications and treatments as prescribed by physicians.

Monitor and document patient progress, ensuring accurate and timely reporting.

Educate patients and their families on proper healthcare procedures and disease management.

Collaborate with physicians and other healthcare professionals to coordinate comprehensive patient care.

Maintain accurate and up-to-date patient records.

Requirements :

Current RN license in the state of Arizona.

Home health experience is prefered, but not required

BLS certification.

Strong clinical and assessment skills.

Comfortable with Wound Care, Wound Vac, Ostomy Care, and Catheter Care.

Excellent communication and interpersonal abilities.

Ability to work independently and as part of a collaborative team.

Reliable transportation and willingness to travel to patient homes.
