Occupational Therapist (OT) – Skilled Nursing Facility (SNF)

Location : Dover, DE

MedAdventures is seeking a skilled and compassionate Occupational Therapist (OT) to join a well-established Skilled Nursing Facility (SNF) team in Dover, DE. This is an excellent opportunity for an OT who is passionate about helping residents improve independence, safety, and quality of life through functional, goal-driven therapy.

In this role, you'll work collaboratively with an interdisciplinary care team to evaluate, treat, and support patients with a variety of rehabilitation needs in a long-term care setting.

    • Key Responsibilities :

  • Evaluate residents and develop individualized treatment plans focused on ADLs, mobility, and safety
  • Deliver skilled occupational therapy services in accordance with care plans and physician orders
  • Collaborate with PTs, SLPs, nursing staff, and families to promote optimal outcomes
  • Educate patients and caregivers on home programs, adaptive equipment, and safe techniques
  • Accurately document services provided, progress, and outcomes

    • Qualifications :

  • Active Delaware OT license (or eligibility to obtain)
  • Experience in a skilled nursing or geriatric setting preferred
  • Strong clinical reasoning, documentation, and communication skills
  • Passion for patient-centered care and functional rehabilitation
