job_description.job_card.job_descriptionProvide physical therapy care in a home health setting. Conduct assessments including AROM, AAROM, PROM, strength testing, and use of assistive devices. Develop, implement, and document patient care plans including long-term and short-term goals. Perform evaluations, reassessments, and plan of care updates. Educate patients and families on home exercise programs and safe body mechanics. Manage patients with orthopedic conditions such as hip fractures, total hip / knee replacements, cervical / shoulder / spinal surgery, low back pain, and osteoarthritis. Treat neurologic conditions including post-stroke rehabilitation, cognitive disorders, multiple sclerosis, muscular dystrophy, Parkinson's disease, spinal cord injury, and traumatic brain injury. Use and train patients on equipment and devices, including lower / upper extremity prosthetics, standard and rolling walkers. State physical therapy license BLS certification Minimum 1 year of experience in physical therapy (home health travel experience preferred; 3 years preferred) Previous travel assignments required (at least 2 preferred) Assessment, evaluation, and documentation proficiency (OASIS, plan of care, reassessments) Experience with patient / family education and home exercise programs Knowledge of orthopedic and neurological rehabilitation Familiarity with assistive devices and prosthetics Vision benefits License and certification reimbursement Continuing Education Medical benefits Wellness and fitness programs Dental benefits Guaranteed Hours Mileage reimbursement Referral bonus Bereavement Holiday Pay Sick pay Retention bonus
