INTAKE COUNSELOR (DEGREED)

Position Summary

The Intake Counselor serves as the first point of contact for referral sources and potential patients to schedule assessments and / or admissions.

Responsibilities include, but are not limited to :

• Thoroughly assessing and completing the assessment tool for all clients.
• Communicating information with the on-call provider and nursing to determine an appropriate level of care.
• Documenting all findings and recommendations.
• Assisting clients in being admitted or referred out to the service or resource to best address their needs.
• Referral management and ensuring prompt response times.
• Providing information to individuals, agencies, facilities, and professional staff (physicians, consulting professionals, etc.) requesting information regarding potential admissions.
• Completing insurance verification and precertification as applicable.
• Participating in crisis and safety interventions as needed.

Qualifications

Education : Master's degree in Social Work, Psychology, Counseling or other related field, or RN required.

Experience : At least one year experience utilizing assessment skills preferred.

Age-Specific Competencies : Adolescent, Adult, Geriatric

Licensure / Certification : CPR required; CPI Certification required.

HIPAA : Has access to Medical Records of cases to which they are assigned work, for quality compliance and risk related issues. Use of PHI is for purposes of Healthcare Operations.
