Summary : This role assists in the delivery of primary health care services and patient management in a fast-paced office setting. Ensure safe and appropriate assessment skills, routine screening, and provide educational instruction on disease management.
Essential Duties : Essential job duties for this position include the following items :
- Performs all job duties / functions according to CCHC mission and values
- Maintains a professional and collaborative work environment to cultivate a positive public image for CCHC
- Participate in care team huddles and pre-visit planning
- Prepare and administer injections and immunizations as ordered per provider
- Monitor patient charts for any medical chart flags / alert and follow-up on outstanding orders in a timely manner
- Greet and room patients in a friendly, respectful manner. Introduces him / herself and the provider to the patient
- Obtains and records vital signs, updates of medical / surgical history, current mediations, and allergy list for each patient
- Assists the provider by reviewing patient charts and identifying any gaps in care and outstanding order according to best practice guidelines
- Assist patients with scheduling routine and follow-up appointments
- Assist the referral coordination team with any medical questions, needs or concerns
- Communicate test results to patients as directed by the provider
- Assist providers during exams and procedures when additional help is needed
- Provide patient education, including medication guidance, home care instructions, and when to seek further medical attention.
- Participates in interdisciplinary medical staff meetings and organizational meetings
- Ability to transition into various roles within department if warranted
- Collaborate with providers, nurses, and support staff to ensure seamless coordination of care
- Recognize and respond promptly to signs of clinical deterioration or emergencies
- Maintain patient confidentiality and always adhere to HIPAA regulations
- Monitor and respond to clinical alerts or patient MyChart messages in coordination with providers
- Support vaccination efforts by screening and scheduling patients for recommended immunizations
- Complete other job duties as assigned

Education / Experience :
Graduation from a Medical Assistant Program or current Iowa LPN licensePrior medical experience preferably in a clinic or ambulatory care setting preferredThis is a full-time position with benefits including health insurance, dental insurance, and vision (company paid) insurance, company-paid life, short-term and long-term disability, voluntary benefits, paid time off, sick leave and holiday, and a 401(k) with 5% company match (no vesting). Schedule is Monday, Tuesday, Wednesday and Friday.
