Position Purpose :
The primary role of a Licensed Practical Nurse (LPN) is to provide comprehensive oversight, coordination, and hands-on care in addressing all health-related matters affecting program participants. This includes both direct and indirect health concerns, ensuring the physical and emotional well-being of individuals through proactive assessment, timely intervention, and ongoing collaboration with interdisciplinary team members, families, and external healthcare providers. The LPN plays a critical role in maintaining a safe, responsive, and supportive environment that promotes optimal health outcomes for all individuals involved in the program.

Patient Care : LPN’s assess patient health, and implement nursing interventions to ensure optimal patient outcomes. Medication Administration : Administer medications and monitor patients for side effects and reactions, ensuring adherence to prescribed treatment plans. Collaboration : Work closely with physicians and other healthcare professionals to coordinate patient care and communicate updates regarding patient conditions. Documentation : Maintain accurate and complete electronic patient records, documenting vital signs, symptoms, and changes in patient conditions. Patient Education : Educate patients about health conditions, treatment options, and self-care strategies to promote recovery and wellness through individual and group sessions. Proficient use of computers, mobile devices, and agency software.
