Flexible start times around 7 : 00 AM, depending on workflow. Monday–Friday schedule — no weekends or holidays required. Charting may be completed remotely after on-site rounds. In-person position based within skilled nursing facilities. Essential Responsibilities Must be Medicaid-enrolled in the state of employment. Provide and oversee the daily clinical care of residents within assigned skilled nursing facilities. Conduct comprehensive histories and physicals, diagnose, and develop treatment plans — including diagnostics, prescriptions, and monitoring. Offer patient counseling and education on health maintenance and disease prevention. Deliver high-quality, patient-centered primary care within the SNF environment. Complete accurate, timely documentation of all encounters. Be available for phone or secure messaging consultations with nursing staff as needed. Use facility EHR systems such as PointClickCare, MatrixCare, and American HealthTech (training provided). Maintain reliable transportation and punctual attendance for all scheduled shifts. Collaborate with supervising physicians when applicable, following established clinical protocols. Participate in chart reviews and quality audits as part of ongoing clinical improvement.
