Contract Type : Contract

Schedule : Monday–Friday, 8 : 00 AM – 5 : 00 PM

Weekend Requirement : None

On-Call : Required every 3 weeks (on-call compensation applies)

Join a compassionate and collaborative home health team serving individuals in the greater Indianapolis area. This RN role focuses on assessment, care planning, disease prevention, and care coordination for patients in home and community settings.

🧠 Responsibilities & Role Overview

  • Perform health assessments and implement individualized care plans
  • Provide care management and coordinate services with other providers
  • Monitor and manage chronic, acute, and preventative health conditions
  • Ensure physician communication, medication administration, and follow-up care
  • Supervise LPNs and unlicensed staff as needed
  • Provide training on health / safety protocols and individual-specific needs
  • Support documentation including progress notes, health status updates, and required nursing paperwork
  • Communicate with inpatient teams during admissions and discharges
  • Promote quality, compliance, and infection control standards

    • ✅ Requirements

  • Active RN license in Indiana or compact state
  • Home health experience preferred
  • OASIS experience preferred
  • AHA BLS certification
  • Valid driver’s license and reliable transportation
  • Must be comfortable with community-based and field care
  • Ability to supervise and educate staff on health / safety best practices

    • Coverage area : Greater Indianapolis\n\nMileage reimbursed at $0.50 per mile\n\nOn-call required every 3rd week ($25 / day weekday, $50 / day weekends / holidays)\n\nScrub colors not specified – professional attire expected\n\nThis is an onsite, field-based role – not remote\n\nCandidates must be dependable, communicative, and confident in independent decision-making
