Your Role

As a Nurse Case Manager , you will play a pivotal role in coordinating care for selected patient populations throughout their acute care journey. Collaborating with interdisciplinary teams, you will enhance patient care by effectively utilizing healthcare resources and leading initiatives to achieve optimal clinical, financial, and resource outcomes.

Key Responsibilities

• Coordination : Integrate healthcare team efforts to meet patient goals.
• Assess patients’ biophysical, psychosocial, and discharge planning needs.
• Advocate for patients and families to secure necessary services and resources.
• Advocacy : Champion patient rights and access to services.
• Work with insurance carriers to obtain benefits and promote treatment goals.
• Report cases for secondary review to leadership as needed.
• Resource Management : Ensure prudent use of resources.
• Evaluate care appropriateness and maintain documentation for third-party payers.
• Accountability : Take responsibility for achieving optimal outcomes.
• Utilize creative problem-solving for complex discharge planning and quality issues.
• Professionalism : Stay informed and maintain competence in your role.
• Educate patients and staff on care levels and regulatory concerns.
• Mentor new staff and uphold patient confidentiality laws.

Additional Information
