Job Purpose
We are seeking a compassionate and dedicated Nurse Practitioner to join our interdisciplinary team within the Program of All-Inclusive Care for the Elderly (PACE). The Nurse Practitioner (NP) will provide primary and specialty care services to PACE participants, focusing on prevention, wellness, and chronic disease management. The NP will work collaboratively with an interdisciplinary team (IDT) to develop and implement individualized care plans, ensuring high-quality and person-centered care.
Duties and Responsibilities
Conduct comprehensive assessments of participants, including physical exams, health histories, and functional status evaluations.
Develop, implement, and evaluate individualized care plans in collaboration with the IDT.Diagnose and manage acute and chronic illnesses, prescribing medications and therapies as needed.Provide preventive care services, including vaccinations and health education.Interdisciplinary Collaboration :Actively participate in IDT meetings to review participant care plans and progress.
Collaborate with physicians, nurses, social workers, and other team members to ensure integrated care delivery.Serve as a clinical resource for the team, providing guidance on best practices and evidence-based care.Care Coordination :Coordinate transitions of care, including hospital admissions, discharges, and specialist referrals.
Communicate effectively with participants, caregivers, and external providers to ensure continuity of care.Compliance and Documentation :Maintain accurate and timely documentation in accordance with program policies and regulatory requirements.
Ensure compliance with state and federal guidelines related to PACE and advanced practice nursing.Participate in quality improvement initiatives to enhance care delivery and outcomes.Qualifications
Education :
Master’s or Doctorate degree in Nursing from an accredited program.Licensure / Certification :Current and unrestricted Nurse Practitioner license in [State].
Certification as an Adult-Gerontology, Family, or Acute Care Nurse Practitioner.DEA license (if prescribing controlled substances).BLS and / or ACLS certification.Experience :
Minimum of [2-3] years of experience as a Nurse Practitioner, preferably in geriatrics, primary care, or chronic disease management.Experience working with diverse and medically complex populations is a plus.Skills and Knowledge :
Ability to work collaboratively with an interdisciplinary healthcare team.Compassionate, patient-centered approach to care, with an understanding of the unique needs of older adults.Strong clinical assessment and diagnostic skills.Excellent communication and interpersonal abilities.Proficiency in electronic medical records (EMR) and documentation.Other :
Must be able to work required schedule.Requires physical strength to perform essential functions of the job.Occasional travel between sites, nursing / group homes and to members’ homes required.Requires use of personal vehicle.Requires valid driver’s license.Requires proof of automobile insurance coverage at the following minimum amounts in order to be reimbursed for mileage : $100,000 / $300,000 personal liability and $100,000 property damage.May require use of personal cell phone for business purposes (may be eligible for stipend)Physical Demands
Must be able to remain in a stationary position 50% of the time.Ability to occasionally move about inside the office to access file cabinets, office machinery, etc.Able to operate a computer and other office productivity machinery, such as a calculator, copy machine, and computer printer.Able to constantly position yourself to maintain files in file cabinets such as reaching with hands and arms, kneeling, crouching, etc.The ability to communicate, detect, converse with, discern, convey, express oneself, and exchange information is crucial for this role.Compliance with the Americans with Disabilities Act (ADA)
