Role Description

This is a full-time on-site role for a Family Nurse Practitioner located in Bloomington, IL. The Family Nurse Practitioner will be responsible for providing comprehensive patient care, which includes diagnosing and treating acute and chronic conditions, conducting routine checkups, and offering preventive care. The practitioner will work closely with the healthcare team to ensure an integrated, patient-centered approach to healthcare delivery.

Qualifications

• Experience as a Nurse Practitioner and in Nursing
• Expertise in Primary Care and Medicine
• Experience in Urgent Care settings
• Strong clinical and communication skills
• Ability to work collaboratively within a healthcare team
• Master's degree in Nursing or related field
• Active Nurse Practitioner license and certification
• Experience in senior and transitional care is a plus
