Job Description
American Traveler is seeking an experienced RN Case Manager with a Michigan RN license, BLS certification, and acute inpatient case management background.
Responsibilities
Position is based in a hospital case management departmentHandles acute care patient population and coordinates discharge planning and utilization managementSchedule consists of four 10-hour shifts per week, 7 : 30am to 6pm, with required weekend rotationTravelers may be required to float to a nearby sister hospitalOnline modules must be completed onsite prior to the start dateResponsible for supporting clinical teams in coordinating patient care, enhancing outcomes, and ensuring efficient utilization of resourcesDuties include precertification, payor authorization, care facilitation, discharge planning, process improvement, and clinical documentation supportWill interact with physicians, payors, patients, and families to remove barriers to timely care delivery and reimbursementCOVID vaccination is mandatoryMust complete background check and drug screening with required documentation by firm due dateNo blocked scheduling is permittedHoliday and weekend coverage may be requiredRequirements
Active and valid Michigan RN license requiredBachelor of Science in Nursing (BSN) requiredAmerican Heart Association BLS certification requiredMinimum of 5 years of acute inpatient case management experience, including acute discharge planningExperience using CarePort system requiredAt least 2 years of recent case management experience requiredPrevious charting experience with AllScripts or GEMS / Sunrise preferredPossess strong communication, negotiation, data management, and PC skillsDemonstrated ability to acclimate quickly and work independentlyBenefits
