- Conduct thorough nursing assessments and collect data using age and area-specific standards.\n
- Create individualized care plans collaboratively with patients, families, and the healthcare team.\n
- Adjust care plans based on patient condition changes and ensure clear communication across team members.\n
- Set realistic, measurable goals for patient outcomes prior to discharge and involve families in the process.\n
- Respect and incorporate the patient's cultural, spiritual, and belief systems in care planning.\n
- Facilitate a smooth discharge and transition to further levels of care with appropriate referrals.\n
- Provide direct patient care consistent with established nursing plans.\n
- Delegate nursing tasks appropriately and ensure task completion by team members.\n
- Continuously monitor the quality of care and ensure safety protocols are adhered to.\n
- Utilize internal and external resources effectively while emphasizing cost containment in practice.\n
- Participate in performance improvement efforts by collecting data and engaging in audits.\n
- Assess and evaluate patient responses to interventions, revising care plans as necessary.\n
- Identify and address patient and family learning needs, ensuring effective education.\n
- Maintain professionalism by reflecting on practices against standards and participating in ongoing education.\n
- Document pertinent information clearly and accurately to support patient care needs.\n
- Act as a patient advocate, coordinating care among providers and team members.\n

- This is a non-management role reporting to a supervisor, manager, or director.\n\nMinimum Requirements\n
- Education : Associate degree in Nursing; Bachelor's degree preferred.\n
- Experience : One (1) year as a registered nurse.\n\nIn Lieu Of\n
- Successful completion of the Prisma Health Nurse Residency Program in place of one year of experience.\n
- One year of LPN experience accepted for internal candidates within the same service line.\n
- Nursing diploma accepted in lieu of an Associate degree with RN licensure.\n\nRequired Certifications\n
