Overview
The Spiritual Care Counselor is responsible for the provision of spiritual care services to patients and families / caregivers of Lake Sunapee Region VNA & Hospice, either directly or through coordination of care with other spiritual counselors.

Assures spiritual assessment of patients and families / caregivers in the hospice program and appropriate services are rendered in a timely mannerFacilitates the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needsProvides direct spiritual care to patients and families / caregiversServes as liaison and support to community chaplains and spiritual counselorsDocuments direct services and ongoing communication with the community chaplains and spiritual counselorsAttends patient care conferences and interdisciplinary group meetings as a member of the interdisciplinary groupAccepts and performs other related duties and responsibilities as requiredQualifications - Per Diem
Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree)Experience working with death and dying, individuals / family / caregiverAbility to work as a member of interdisciplinary groupComfort in an interfaith settingAbility to accept different lifestyles, cultures, beliefs, and valuesAbility to network with community clergy and congregationsKnowledge of and commitment to hospice philosophy of careProof of Flu vaccinationReliable transportationValid driver\'s licenseAutomobile insurancePhysical exam within the last yearTwo-step TB testLake Sunapee Region VNA & Hospice is a mid-sized agency, providing home health care, hospice, palliative and personal care services to people of all ages in parts of Grafton, Merrimack, and Sullivan counties in New Hampshire.
