Position Overview :
Seeking a Licensed Practical Nurse (LPN) or Registered Nurse (RN) with specialization in wound care . The nurse will perform wound assessments, manage care plans, provide education to staff, and ensure residents’ skin and wound health in compliance with facility policies and clinical standards.
Key Responsibilities :
- Perform wound assessments, reassessments, and management for all residents.
- Develop skin care programs to prevent skin breakdown.
- Implement policies for management of wounds, fistulae, tubes, ostomy care, and incontinence.
- Revise care plans based on resident response and treatment effectiveness.
- Assess and manage pain; educate residents and families on pain management.
- Serve as a resource to nursing staff; provide in-services on wound care, ostomy care, and incontinence.
- Communicate effectively with nursing staff, physicians, and interdisciplinary team members.
- Ensure accurate documentation according to current standards.
- Stay current with training, continuing education, and best practices in wound care.
Required Qualifications :
Active LPN or RN license in Arizona.Certification as a CWOCN (Certified Wound, Ostomy, Continence Nurse) or CWCN (Certified Wound Care Nurse).Ability to obtain and maintain Arizona DPS Fingerprint Clearance Card .Current CPR / BLS Certification .Local candidates only – available for in-person interviews and start within 2 weeks of offer.Preferred Qualifications :
