Job Title : Home Health Aide for Nursing Students

Hours Per Week : Per Diem

Schedule : Sunday through Saturday - Day and evening shifts available. Must be available to work a minimum of 1 - 8 hour shift every 2 weeks with 1 weekend shift per month.

As a nursing student, this position provides valuable patient care experience. You'll be responsible for caring for clients according to their individual care plans, demonstrating compassion and making a positive impact each day.

ABOUT ELDERONE

ElderONE, an affiliate of Rochester Regional Health, is the area's only Program of All-Inclusive Care for the Elderly (PACE). We serve Monroe, Ontario, and Wayne counties, offering a comprehensive range of medical care and support services for seniors aged 55 and older with chronic care needs, enabling them to live independently at home without sacrificing their health or safety.

RESPONSIBILITIES

• Assist in implementing a comprehensive care plan to meet both daily and long-term needs of patients.
• Keep accurate records of patient activities and their overall condition.
• Support patient routines with activities that include personal care, light housekeeping, and meal preparation.
• Timely complete and submit activity sheets as per department policies.
• Participate in a minimum of 12 hours of annual in-service education / competencies.
• Report any changes or concerns regarding the patient’s condition to appropriate staff promptly and document the information properly.
• Keep family members and staff updated on clients’ activities and any concerns.
• Perform additional duties as required.

REQUIRED QUALIFICATIONS : You must be a nursing student who has successfully completed coursework covering home health aide tasks within the last 24 months. An official transcript is needed to verify completion of Fundamentals in Nursing.

Required Licensure / Certification : A valid NYS Driver's license is preferred. Completion of the written and one-day HHA Skills competency program provided by Rochester Regional Health is mandatory, and you must achieve active New York State HHA certification within 4 days of hire.

PHYSICAL REQUIREMENTS : Medium Work - Ability to exert 20 to 50 pounds occasionally, 10 to 25 pounds frequently, and more than negligible amounts constantly. This position requires frequent walking, standing, or squatting.
